Confidentiality policy
1. Purpose

This Confidentiality Agreement is established to ensure that the information shared between the Psychologist and the Client remains private and secure, in accordance with ethical and legal requirements. Non of the information exchange during our sessions would be disclosed without clients previous consent.

2. Confidential Information

Confidential Information includes, but is not limited to:
• Psychological assessments, diagnoses, and treatment plans.
• Personal and medical history disclosed during sessions.
• Session notes and progress reports.
• Any communication between the Client and Psychologist.

3. Confidentiality Obligations

The Psychologist agrees to:
• Keep all Client information strictly confidential.
• Only disclose Client information with the Client’s written consent, except as required by law (see Section 4).
• Securely store all records and ensure compliance with privacy regulations.

4. Exceptions to Confidentiality

Confidentiality may be breached in the following circumstances:
• If the Client is at imminent risk of harming themselves or others.
• If there is a legal obligation to report abuse, neglect, or exploitation.
• If required by a court order or legal investigation.

5. Client Rights

The Client has the right to:
• Access their records upon written request.
• Withdraw consent for information sharing at any time, except where legally required.
• Expect professional and ethical handling of their personal information.

6. Duration of Confidentiality

This agreement remains in effect throughout the professional relationship and continues after the termination of services, as required by applicable laws and regulations.
